2014-15 PTA Membership Form (once complete, form & payment may be turned in to the PTA box in the main office)
Request for PTA Funds Form (All new allocations must be pre-approved by PTA Board and/or Membership)
New Project Request form Requests must be submitted to the PTA Board by the 25th of the month prior to the next scheduled board meeting in order to be considered.
ODE Criminal History Form (required once every 2 years for volunteering, at school, with Sabin students)
