We need loving and dedicated volunteers to help out in our Kelly Community.  Please indicate how you would like to be involved at Kelly this year by checking off the items below and on the other side.  Remember that even an hour of your time can make a difference to your children and your community!

NOTE: We only need one form even if you have more than one student.  Also, if your family has more than one volunteer, please either fill out a separate form for each volunteer or identify different volunteers with initials.
