Duniway Elementary School

Enrolling at Duniway is a simple process that can be done in person at our school office. If Duniway is your neighborhood school, you will need to gather the items listed below. All five are required for enrollment. Please feel free to call us if you have any questions.

1. A completed Student Registration form. A copy of the Student Registration form may be found here.
2. Evidence of immunization for diphtheria, tetanus, polio, hepatitis B, chicken pox, Hib, mumps, rubella, and measles. These are required unless parents wish to seek an exemption because of medical or religious reasons.
3. Proof of age for the student. A birth certificate or other similar document is sufficient. Children must be five years old or older by September 1 to enroll in kindergarten. Children must be six years old or older by September 1 to enroll in first grade.
4. Proof of home address. Two or more documents such as utility bills, rental agreement, or other bills to the home address. These must be recent and the name must be of a parent or guardian of the student.
5. Contact information for a previous Public School attended (if applicable).
